Constantia Care provides a serviced which is tailored to the needs of the individual client who can no longer manage to maintain themselves without support within their own home. Our services are generally provided to the following people:

Assessment
We will carry out a face to face assessment, work out your care needs, and match the best care assistant to suit your live-in care requirements. This is then incorporated into a person centred care plan, which is compiled by the agency. This care plan gives the live-in care assistant an insight into the clients’ background and medical history and enables the carer to interact with the client on a personal basis, giving the essential care and support required.
Looking Back
Reminiscing is something we all do. The older we get, the more likely we are to fall back on our memories to generate those sought after happy moments; regardless of whether we are alone or socialising with friends and relatives. Reminiscence is a perfectly natural activity which can have a great therapeutic effect. Triggers can often bring back those memories that reveal a rich stream of insight and experience that is otherwise hard to bring to the surface. The person-centred care plan will give the care assistant those triggers and allow the a meaningful relationship to be formed with the client.
Companionship
You may not have family or friends who live in close proximity to assist you with your general daily needs, like shopping and doing everyday tasks. Nutrition
Food is an essential part of any successful care. People generally love food, it is a social activity and provides obvious health benefits. The face to face assessment of our client will incorporate an in depth section of likes, dislikes and specific dietary requirements. This will be incorporated into the person centred care plan. The live-in care assistants are trained in nutrition and good food hygiene. All of our home care assistants are good wholesome cooks.
