Office Support

Constantia Care is available to take your call at any time, day or night, ensuring support to the client, their family and the carer.

Our office line is available 24/7 with at least three members of our team covering any of the out of hour lines to offer support and guidance should you need it.

Each care co-ordinator has an office line, accessed by calling our direct line and also a work mobile which the client can use to text or email them at any time.

The Role of a Care Co-Ordinator:

  • Liaise with clients and carers on an ongoing basis
  • Daily updates are sent through to the care coordinators to input into our mainframe, so that at any time we remain current with our clients’ needs
  • Provide telephone support on a daily basis
  • Arrange any holiday or sick cover that is required
  • Informing the client and or family of any changes coming up and how this will translate to the care provided
  • Perform 6-8 week spot checks, informing the client / family they are coming in advance to ensure this is convenient
  • Update care plans and risk assessments as required and review plan of care at the spot check
